Serotonergic Drugs
Combining haloperidol with serotonergic drugs, such as selective serotonin reuptake inhibitors (SSRIs; e. g., fluoxetine, sertraline) or serotonin-norepinephrine reuptake inhibitors (SNRIs; e. g., venlafaxine, duloxetine), can lead to serotonin syndrome, a potentially life-threatening condition characterized by agitation, confusion, hyperthermia, and rigidity. Careful monitoring and consideration of alternative treatments are crucial.
Lithium and other antipsychotics should also be used cautiously with haloperidol. These combinations increase the risk of extrapyramidal symptoms (EPS), such as Parkinsonism and tardive dyskinesia. Regular monitoring for EPS is necessary. Adjust dosages cautiously, and closely monitor the patient for any adverse effects.
Drugs that prolong the QT interval, such as certain antiarrhythmics and certain antibiotics, should be used with extreme caution. Concurrent use can significantly increase the risk of torsades de pointes, a potentially fatal cardiac arrhythmia. ECG monitoring may be required.
